great! Could you please clarify how heels can relieve pain in fat pad ?where , it is more painful in extension and, wearing heal would take knee into further extension. thank you in advance.

Most high heels will increase the amount of knee flexion and hence the patients with PFP tend not to like it, but the fat pad patients often like the flexion. Here is a link to a paper that may help: http://www.sciencedirect.com/science/article/pii/S0966636211008009
